Normal Challenges "Slay Bloodgred Spiders with Demon Powers" located in Dark Ages - only Demon Bloodgred contributes towards this challenge, I got around this by dying once I killed the Demon Bloodgred so that his Monument of Rock reset and I could kill him again.
"Slay Tyrant Succubus Swordmaidens" located in the Screaming Pathway. These simply don't spawn, however once I completed every other normal challenge this unlocked despite being 0 out of 3 before and I got the achievement.
Elite Challenges "Slay Champion Corrupted Norns with Melee Weapons with 5 Hex" located in Frozen Hearth Cave, did this one at least twice and both times no progress.
"Slay Tyrant Succubus Swordsmaiden with Hex of Malice" and "Slay the Sisters of Mercy without Using Potions with 5 Hex" located in Screaming Pathway. Did both of those several times and no progress.
"Slay Succubus Swordsmaiden Champions with Melee Attacks without Dying" located in Dead Man's Mine. There are no Swordsmaiden Champions there, just Swordsmaiden.
