For this trophy, you'll need to play as the Tainted Coil in a multiplayer match against Ironheade. Once the match starts, upgrade your own stage once, spawn a Warfather, then double team with him to spawn a Pain Lifter. Once your opponent's got a large crowd of units (preferably Headbangers and Razor Girls that clump together) ready and waiting, drive the Pain Lifter over to their part of the map, then aim and fire an Agony Ball into the center of the crowd - you should be able to easily hit 15 since the blast radius is pretty large.
As with the other four unit-specific trophies, this can be earned offline by playing against an AI, but it is much, much easier to aim for with a boosting partner who can spawn lots of Headbangers and have them simply mass and wait in front of the stage while you make your preparations and eventually attack them.
